Revenue Codes for Hospitals, Emergency Rooms and Facilities with UB04 Billing 2021

Revenue codes are 4-digit numbers that are used on hospital Institutional bills to tell the insurance companies as where the patient was when they received treatment, or what type of service or item a patient might have received as a patient. A medical Institutional claim will not be paid if Revenue Codes are missing from the bill.

What is a revenue code on a UB04?

Revenue codes are 4-digit numbers that affect reimbursement and represent the services provided by the ASC facility for a payer. Note that you can’t report revenue codes on a CMS-1500 form, but you can report them on a UB-04.

What is the revenue code for treatment room?

Revenue code 0761 is for a treatment room and should not be used in place of an observation room.

What is the revenue code for Observation hours?

Revenue code 0762 is for a Observation hours, most of the payers may require minimum of 8 hours and shouldn’t exceed 24 hours but may extend to 48 hours based on medical necessity, but no longer than 72 hours without a discharge.

What is the revenue code for Emergency Room?

Revenue code 0450 is for a Emergency Room including Freestanding Emergency Rooms, and commonly used for ED Levels 99281-99285.
